Referral System
Please carefully read our simple three step guide to help you understand our referral system. Please remember that you do not need a specific referral from a GP or paediatrician to access OT services!
Step 1
Download the appropriate form for the participant/client you would like to refer to us. There are two separate forms:
- Form one is for NDIS participants only
- Form two is for all other non NDIS clients
Step 2
Fill out referral form:
- If you encounter issues downloading the referral form or have questions regarding the content within the form
- Please email us at admin@mabhenatherapy.com or reach out to us on 0457 205 006
Step 3
Email the completed form to admin@mabhenatherapy.com
- Please attach any relevant reports in this email
- Clear photos or scans of the completed referral form are recommended
- Once this has been submitted, our friendly team will be in contact as soon as possible
